To create this collection of summaries of promising models of care, Geoff Anderson, M.D., Ph.D., and colleagues at at the University of Toronto worked with policymakers, thought leaders and researchers in 11 countries to identify innovative programs that provide integrated health and social care to high-need high-cost populations.  They used a structured survey to collect information.
