Participate in a Medical Homes Summit
<p>On behalf of the National Academy for State Health Policy (NASHP) and the Patient Centered Primary Care Collaborative, The Commonwealth Fund invites states to apply to participate in the Medical Homes Summit, to be held July 24-25, in Washington, D.C. This Commonwealth Fund-supported summit is designed to help state policymakers improve the quality and availability of medical homes for Medicaid and State Children Health Insurance Program (SCHIP) beneficiaries.<br><br>The summit will host teams from up to 10 states that are developing or have committed to developing medical home initiatives.
