<p>The Commonwealth Fund has asked Rutgers University Center for State Health Policy to conduct an assessment of its Health System Performance Scorecards in order to improve the Scorecards to better meet your needs. The Fund Scorecards are an online resource for states, communities, and the nation that provides health system performance benchmarks and rankings of healthcare access, quality, efficiency, and health outcomes.</p>
<p>This confidential, online survey of potential audiences for the Scorecards will take about five minutes to complete. Your participation will contribute to making the Scorecards useful to people like yourself.</p>
