Building on the work from the first two phases, the project team will work closely with selected delivery systems and implementation partners to analyze, model, and project the potential impact of an integrated set of U.S. and international care innovations matched to their problems. The analysis will enable more data‐driven decision-making for implementation of a solution set. The solution sets will combine U.S. and international approaches that offer innovative strategies to address key challenges that they face in caring for high‐need, high‐cost patients.
