Informed by earlier Fund-supported focus group research (conducted by SSRS), the new Commonwealth Fund Delivery System Reform Survey will collect data on a range of topics, such as: communication between care providers and patients; the doctor–patient relationship; integration and coordination of care among providers, including social service agencies; and patients’ difficulties in navigating the health system. The survey research firm SSRS will collect data through its Omnibus survey, which will yield a nationally representative sample of 3,000 adults, including an oversample of 1,800 people with multiple chronic conditions or functional limitations affecting daily living. The Fund-designed instrument will be in the field from approximately June 1 through August 31, 2016.
This survey will produce timely information about how high-need, high-cost individuals experience the health care system. Dissemination activities will target the national media, policymakers, health system leaders, and payers.
