This grant will continue support for the unique peer-to-peer learning collaborative known as Promoting Integrated Care for Dual Eligibles (PRIDE), whose members—seven small, high-performing health plans—are seeking to address the operational challenges of creating integrated care systems for people enrolled in both Medicare and Medicaid. Results from the first two project phases have informed the federal contracting process for integrated care plans taking part in various federal demonstrations; they also have assisted local health plans with locating beneficiaries and conducting comprehensive patient needs assessments. In the third phase, participating plans will develop and test concrete strategies to address common challenges identified in earlier phases.
