This project will convene and support two separate learning communities, one for Medicare Advantage (MA) plans and one for Medicare accountable care organizations (ACOs). Partnering with the Accountable Care Learning Collaborative, the Center to Advance Palliative Care (CAPC) team will recruit and facilitate a learning community for at least 12 ACOs. For the MA learning community, CAPC will recruit at least 15 participants from its multipayer workgroup, which includes several key MA plans, and work with plan associations such as the Blue Cross/Blue Shield Association, the Association of Community Health Plans, the Association of Community‐Affiliated Plans, and the SNP Alliance (special needs plans).
