This project will provide international examples of promising interventions that address social isolation and/or loneliness to improve health to inform development of similar programs or policies in the U.S. among frail elderly/high-need adults. To build the repository of programs and interventions, the social isolation team will first conduct a literature review. The team will then compile a list of U.S. and international experts identified from the literature review and conduct informant interviews. Through the literature review and informant interviews, the team will determine five programs that meet their criteria for inclusion to conduct a deep-dive analysis on program characteristics in order to inform innovative ideas for interventions to address social isolation among the frail elderly/high-need patients.
