The project team is partnering with the National Center for Complex Health and Social Needs to convene a 12-month accountable care organization (ACO) learning community. The primary goals of the learning community are to: 1) help 10 to 15 ACOs implement programs that improve patients’ medical and social well-being; 2) build an effective learning platform and resource library to help ACOs not participating in the project implement their own initiatives; and 3) develop knowledge around the factors that enable effective implementation of this type of initiative.
