The Meadows Institute will develop and disseminate policy and practice recommendations to support the implementation of a collaborative care model (CoCM) in Medicaid for transition-age youth (ages 14–25). Activities include: Phase I — Conduct a national scan of state Medicaid programs to produce a 50-state overview of Medicaid CoCM financing and adoption, including codes and rates currently available for reimbursement, rates compared to Medicare, medical policy requirements, and rates of utilization (if available). Phase II — Identify four to six regions that have successfully implemented CoCM; conduct structured interviews with key stakeholders from each region; synthesize results into a paper on specific barriers and facilitators for implementation.
