This project will support four issues of the newsletter Transforming Care, three case studies, and one issue brief. Transforming Care topics will be developed in collaboration with Fund staff. Each issue will include a feature article; a case study or interview with an expert on the topic; infographics; and a roundup of important journal articles identified by the editorial advisory board. Potential topics include health care providers’ role in suicide prevention, use of social media in health care, and impact of initiatives seeking to integrate health and social services. The proposed case study topics, developed in partnership with program staff, are: 1) Rhode Island’s efforts to strengthen primary care and reduce costs; 2) rural hospitals and population health; and 3) how Centene – one of the nation’s largest Medicaid managed care companies – is building a system to identify the most effective and appropriate social services providers and track their impact on members’ health.
