The Urban Institute’s Bradford Gray will produce case studies of innovative models from abroad that show promise for improving care for high-need, high-cost patients—in particular, frail elderly, chronically ill, and mentally ill individuals. The four programs to be profiled are Sweden’s Senior Alert and Esther Model, the Netherlands’ ParkinsonNet, and the U.K.’s Big White Wall. Three of these are currently being piloted in the U.S. The research team will conduct literature searches, site visits, and interviews with experts in each country. The case studies will inform U.S. policymakers, providers, researchers, and journalists about successful strategies used abroad to improve health system performance.
