The purpose of this project is to engage and increase coordination among national, regional, and local funders who are interested in improving care for people with complex health and social needs. Grantmakers in Aging (GIA) and Grantmakers In Health (GIH) will engage funders by disseminating and sharing evidence‐based resources, including those from the six-foundation Playbook, as part of a larger communications strategy. The major activities for achieving this objective include offering learning and collaborative opportunities through webinars and at GIH and GIA workshop conferences as well as monthly dissemination of educational materials and Playbook promotion through GIA’s network.
