CHRT proposes to develop and disseminate up to three issue briefs that will detail lessons learned and policy implications of topics that are relevant to policymakers, providers, payers, and interested members of the public across many states. Each of the issue briefs will address a unique gap in information and the three original topics proposed are: the sustainability of a multi-payer advanced primary care practice demonstration project; the integration of community health workers (CHWs) in the State Innovation Model (SIM) initiative; and the impact of Medicaid expansion on community mental health systems. CHRT will work with the Fund to revise the paper topics, especially in light of the results of the 2016 national elections, as necessary. CHRT will conduct outreach to disseminate the three issue briefs and will work collaboratively with the Fund and other organizations on joint dissemination efforts.

The three issue briefs will be targeted to state policymakers around the country, highlighting approaches to sustainability of demonstration projects, advances in the evidence base and best practices for employing CHWs, and care, coverage, and funding lessons learned through Medicaid expansion. The targeted and timely dissemination of each issue brief will encourage states to learn from one another and ensure that state policymakers have information needed to make evidence-based decisions.
