The Commonwealth Fund-supported Care Transitions Search Widget is a customized search tool that provides efficient access to high-quality information on improving care transitions and patient safety. The widget was designed by Growth House, Inc., using Google's custom search technology. It prioritizes search results based on a carefully selected list of Web sites that focus on evidence-based quality improvement for health care. This free tool can be added to any Web site and resizes content for display on mobile phones and tablets.
The Care Transitions Search Widget’s primary distribution site is at http://medicaring.org. The widget is also running on 11 additional partner sites, including the Colorado Foundation for Medical Care and the Long-Term Quality Alliance.
The search function goes first to a list of highly reliable sources and then adds entries from the rest of the Web if the initial search results are thin. The search engine has tabs for filters to help the user narrow search results. For example, a person who started out searching for “patient activation” could then filter results by “hospital readmissions.”
