A new issue brief explores "virtual wards," in which the systems, staffing, and daily routine of a hospital ward are used to deliver preventive care to patients in their own homes. In this manner, coordinated case management services are provided to people who are at high risk for emergency hospitalization, as identified through predictive modeling. This model has been deployed in a number of locations in the United Kingdom and other countries, including the United States.
