The latest issue of Quality Matters reports on Independence at Home, a federal demonstration project that is testing whether treating frail, elderly patients with multiple chronic conditions in their homes improves outcomes and reduces health care spending. While the three-year initiative is only at its midpoint, some of the participating medical practices have already reduced hospitalizations and readmissions dramatically by providing more intensive services to these patients.
