The National Academy for State Health Policy (NASHP) has launched a new interactive map tracking state efforts to advance medical homes for enrollees in Medicaid and the Children's Health Insurance Program. Map users can explore payments to medical homes, alignment of payments with qualification standards, multipayer initiatives, health homes created under the Affordable Care Act, and use of shared practice supports, such as community-based teams. NASHP has been tracking and supporting state medical home activity since 2007 with the support of The Commonwealth Fund.
