Improving Chronic Illness Care in Large Practices

This Commonwealth Fund–supported study investigated the extent to which physician practices use evidence-based care management processes to control their patients' chronic conditions. Based on a national survey of large physician practices, it found that, on average, such practices use about half of recommended care processes, most commonly disease registries, patient educators, and performance feedback. Higher use was correlated with participation in quality improvement programs, having a patient-centered focus, and ownership by a hospital or health maintenance organization..
