Even with financial incentives to reduce readmissions in place, many hospitals are not implementing all of the strategies that have been shown to work, according to this Commonwealth Fund–supported survey of hospitals participating in the national Hospital to Home initiative. The survey found that more hospitals were discharging patients with follow-up appointments already made and were formally estimated patients' risk of readmission. Still, many strategies—including contacting community physicians about discharges and following up on test results that came back after patient discharge—were used by less than 40 percent of hospitals.
