Many older adults have complex health problems that are exacerbated by nonmedical issues such as unsafe housing, lack of transportation, and isolation. Commonwealth Fund–supported researchers identified four factors common to “high-performing communities” where coordination between health care providers and local social services organizations has helped control medical care use and costs. The study, in Health Services Research (Sept. 19, 2017), was led by Amanda Brewster of the Yale Global Health Leadership Institute.
