A new approach to proactively managing care for high-need patients in one region of England has led to a 13 percent reduction in emergency department (ED) use. The Extensive Care Model, piloted in the NHS Vanguard region of Fylde Coast, targets adults age 60 and older who have two or more chronic illnesses. The model provides them with assessments by a group of medical and social service providers, who then develop and carry out comprehensive care plans.
