A number of state Medicaid agencies are partnering with Medicare to integrate care for “dual eligibles,” who are enrolled in both programs. Two briefs published by the Center for Health Care Strategies with support from The Commonwealth Fund and The SCAN Foundation identify lessons from these efforts. The authors focus on the development of interdisciplinary care teams to meet beneficiaries’ medical, behavioral, long-term care, and social service needs and the best ways to share results with stakeholders.
