Case Study Series: Models for Aging in the Community and Hospital Care at Home

Our series on care models for high-need, high-cost patients continues, with two new case studies:

  • Aging Gracefully: The PACE Approach to Caring for Frail Elders in the Community” tells the story of San Francisco’s On Lok Senior Health Services, the first PACE organization in the United States. PACE, or the Program of All-Inclusive Care for the Elderly, has helped thousands of frail elders across 31 states “age in place” at home and remain engaged with their local community.
  • The Hospital at Home Model: Bringing Hospital-Level Care” looks at a program at Presbyterian Healthcare Services in Albuquerque, N.M., where teams of physicians, nurses, and other clinical staff make house calls to elderly patients, providing hospital-level care at home for conditions that can be safely treated there.
