With Commonwealth Fund support, the Center for Health Care Strategies (CHCS) studied a pilot program developed by the Health Plan of San Mateo in California that helps Medicare–Medicaid enrollees in nursing homes transition to community living. A CHCS profile of the program provides lessons that may help other health plans, providers, and states with their own approaches to care for adults needing long-term services and supports.
