Unexpected hospital readmissions are expensive and challenging for patients and their families, and costly for Medicare. With support from The Commonwealth Fund, researchers led by Yale’s Elizabeth Bradley examined the impact of two national efforts to reduce readmissions, State Action on Avoidable Rehospitalizations and Hospital-to-Home. In a new blog post, Bradley and colleagues highlight which strategies were most effective in helping to lower hospital readmission rates.
