This one-year feasibility study will seek to determine if interested communities are ready to implement a financing model for social services integrated with health services, with the explicit intent of improving patient outcomes and lowering overall costs of care. The researchers will examine which local conditions — such as data capacity, presence of coalitions, or prioritization of social determinants of health — facilitate a community’s readiness and likelihood of success.
