A randomized controlled trial will test a hybrid primary care intervention that combines community health workers (CHWs) and digital health tools to help low‐income people with poorly controlled diabetes. Patients will be assigned to one of three groups: a group that uses digital health tools only; a group that uses digital health tools and receives visits from CHWs if patients are struggling with care management; and a group that receives usual primary care (no CHWs or digital tools). The three patient groups will be compared over a 24‐week period for rates of adherence to daily glucose self‐monitoring and for overall diabetes control.
