This project will develop actionable guidance for states to assist them in designing and implementing public health-primary care (PH-PC) integration efforts. The team will conduct a landscape analysis highlighting current PH-PC state-level integration efforts; interview approximately 30 leaders, experts, providers, and public health practitioners in three leading states on PH-PC integration (North Carolina, Rhode Island, and Washington) to produce in-depth case studies; develop a case study of a nascent state on PH-PC integration (Michigan); and develop a state toolkit summarizing lessons and strategies. Throughout these activities, they will focus on the three domains of PH-PC integration: fostering cross-sector knowledge, investing in shared data systems, and creating and aligning incentives for integration.
