This project provides general support for the New York City COVID-19 Rapid Response Coalition. The current phase of work is focused on designing and implementing a sustainable and scalable delivery model, which will include using patient engagement technology and processes, coordinating clinical and social services, and strengthening existing community-based social services and clinical programs. The Coalition aims to launch the model and reach more than 30,000 vulnerable New Yorkers by June 30, 2020. Public Health Solutions will continuously measure the short-, medium-, and long-term impact on participants’ health, quality of life, and costs of services.
