This project will use a multisite case study approach to determine whether there are differences in access to postacute home health care services and health outcomes between those with traditional Medicare and those enrolled in Medicare Advantage plans. The sample selection will consist of recruiting individuals from different organizations within six target markets which will vary by region and Medicare Advantage penetration. Interviews will be conducted with hospital discharge planners, case management leadership, home health admissions directors, and administrators from about 36 organizations (six organizations selected from six different markets).
