Drawing from their previous research, the investigators will lead a process to codesign a community‐based primary care intervention among Mount Sinai and its community partners in Harlem. The team will first recruit and convene advisors and stakeholders from the Harlem community to choose one challenge to improving health for vulnerable populations in their neighborhood. The project team will then hold six workshops with health system leaders and community partners to identify components of successful primary care models from other countries that could address the challenge. A guide will be developed to facilitate implementation of these components; for example, it might show how to deploy a care team that include clinicians, community health workers, and social services staff. The implementation guide will detail all key steps, provide checklists, and describe necessary changes to IT systems, staff roles, and workflow.
