Co-directed by Donald Goldmann, IHI chief medical officer, and Fund vice president, Robin Osborn, Phase Three of this project will pilot-test and evaluate the transferability to the U.S. of four frontline delivery system innovations sourced from abroad to address the following problems: overuse of low-value tests and treatments; shortfalls in patient and family experiences of care; gaps in care coordination across settings and the care continuum; and, poor support for patient and family engagement in self-management. Using the infrastructure established in Phase One and the firsthand knowledge of the solutions gained in Phase Two, the 15 delivery systems in the Innovators Network will test, evaluate, amend, and spread the selected innovations in their delivery system, with coaching and technical assistance from the IHI team. Throughout Phase Three, Innovators will participate in a shared learning network and evaluation protocol. IHI and the Fund will collaborate in disseminating the most effective innovations to health care delivery systems nationwide.
