The Institute for Healthcare Improvement (IHI) will collect, curate, and package resources for the playbook in preparation for a fall 2016 launch. Components will include: a value proposition for payers and providers; a framework for segmenting the HNHC population; patient profiles developed with qualitative and quantitative information; case studies of evidence-based models; and a return-on-investment calculator. An advisory group of ACO and Medicare Advantage plan leaders will consult on the website’s conceptualization, design, and functionality. IHI will subcontract with a web developer to design and build the playbook and provide data on the site’s audience and traffic. If successful, the collaborative will seek additional funding to expand the playbook’s content to support clinicians and managers in implementing best practices.
The playbook will help providers and payers identify and select evidence-based models to improve outcomes and lower the costs of caring for some of the health system’s sickest, most complex patients.
