This project will support the Playbook’s management and development in 2022 and 2023. The goals continue to be to: 1) increase access to resources on evidence-based and promising practices in complex care; 2) promote adoption of these practices; and 3) profile health care organizations at the forefront of complex care. For phase 6, the Center for Health Care Strategies (CHCS) will showcase effective implementation strategies through first-person perspectives and practical resources and create new venues for peer-to-peer sharing (e.g., deep-dive webinars and virtual site visits). The CHCS team also will partner with the Tradeoffs podcast to produce up to three episodes on complex care. All work will focus more on racial equity.
