We explored findings on such topics as: provider preparedness to manage patients with complex conditions, practice capacity to provide chronic care management, care coordination, and more. Experts from the Netherlands and the United Kingdom offered their perspectives as well. Findings from the survey, which also included Australia, Canada, France, Germany, New Zealand, Norway, Sweden, Switzerland, and the United States, were published by Health Affairs in December.
